Chapter 4 - The Subterranean Ward

The heavy iron door beneath the north wing of the estate groaned as Jack shoved the rusted key into the lock and twisted. The deadbolt gave way with a sickening crunch of grinding metal.
Jack clicked on the tactical flashlight mounted beneath the barrel of the SIG Sauer 9mm he had pulled from his duffel bag. A gust of stale, chemical-laden air hit him squarely in the face: isopropyl alcohol, bleach, and the unmistakable metallic reek of dried blood and industrial disinfectant.
He descended the stone steps into what had once been an old Prohibition-era bunker beneath the mansion. As the flashlight beam swept across the concrete floor, Jack felt his stomach heave.
This was no wine cellar.
It was a clandestine surgical suite. Two overhead surgical lights hung from the raw concrete ceiling, connected to a noisy generator humming in the corner. In the center sat an adjustable operating table equipped with leather restraint straps—all four straps were heavily stained with dark, dried fluid. Nearby stood an apheresis machine, its plastic collection bags crusted with old plasma, flanked by stainless steel trays holding bone marrow biopsy needles, large-gauge trocars, and surgical scalpels.
Jack walked slowly toward a desk against the far wall. Piles of manila folders lay scattered across the surface, stamped with the Vance Family Foundation seal.
He opened the top file.
Subject: Elena Vance (née Rostova). Blood Group: O-Negative. Full HLA match for Recipient: Liam Vance.
Jack flipped the page. Photos fell out onto the desk.
Photos of Elena bound to the table. Photos of needle aspiration marks along her posterior iliac crest—dozens of punctures, red, inflamed, and weeping fluid. The notes in the margin were written in meticulous, elegant handwriting:
Harvest #1: 1200ml marrow aspirated. Subject experienced hypovolemic shock. Administered ephedrine.
Harvest #2: 800ml marrow aspirated. Subject resisted. Administered intravenous propofol without anesthesiologist monitoring.
Harvest #3: Neurological distress noted. Trepanation performed to relieve intracranial pressure caused by blunt head trauma sustained during physical restraint.
Jack gripped the edge of the desk, his knuckles turning dead white. They hadn't just taken marrow; they had beaten her when she fought back, then drilled into her skull in a filthy basement to keep her from dying before they finished stripping her marrow for Liam.
Suddenly, the hum of the basement generator sputtered and died. The surgical suite was plunged into absolute, suffocating darkness.
From the top of the concrete stairs, the heavy iron door slammed shut, followed by the definitive, metallic slide of an exterior electronic deadbolt.
