Chapter 2 - The Cold Light of Deceit

Dr. Marcus Vance stood over the stainless-steel examination table, wiping condensation from his wire-rimmed glasses. The emergency room bay was unusually quiet for a Friday midnight, save for the rhythmic beeping of vitals in neighboring stalls and the muffled sound of rain lashing against the tinted glass doors.
A private ambulance team had bypassed the standard municipal dispatch, wheeling the gurney through the rear service bay.
""Blunt force trauma, severe cranial laceration, non-responsive,"" the lead paramedic recited in an emotionless tone that bordered on scripted memorization. ""Found at the base of the staircase by property staff.""
Marcus moved quickly, his fingers checking the pupillary reflex. Elena’s right pupil was blown, her skin clammy and grey. The crimson soaked into her blonde hair was already clotting. ""Get her into trauma one! Prep two units of O-negative and call neurosurgery immediately!"" he barked at the attending nurses.
""There won’t be a need for surgery, Doctor.""
The voice cut through the chaos like a scalpel. Marcus turned to see Marianne standing near the entryway, wrapped in an expensive charcoal cashmere coat. Her silver hair was smoothed back into place. Beside her stood Arthur Bradley, the family’s longtime estate attorney, holding a sealed black leather dossier.
""Mrs. Sterling,"" Marcus said, his jaw tightening. ""Your daughter-in-law is suffering from massive intracranial hemorrhage. If I don't get her into the OR within twenty minutes, she will die.""
""My daughter-in-law was unstable, Dr. Vance,"" Marianne said, her tone level and silky smooth. ""She took her own life. In her psychosis, she abandoned all parental responsibility and threw herself down the stairwell. Her DNR is already on file, signed and notarized last month.""
Bradley stepped forward, extracting a single sheet of paper stamped with a legal seal, holding it directly before Marcus's eyes.
""This is absurd,"" Marcus spat, stepping around the lawyer to reach for the resuscitation cart. ""She's twenty-eight years old! And where is the child? Where is Leo?""

Marianne’s eyes hardened into chips of obsidian. She stepped into Marcus's personal space, lowering her voice so only he could hear. ""The child is with his legal guardian. And you, Marcus, have a daughter waiting on the donor registry for a specialized pediatric liver transplant. One phone call to the hospital board ensures she reaches the top of the list by morning. Another phone call ensures you lose your medical license and your daughter dies waiting.""
Marcus frozen, his hands trembling inches above Elena's chest. On the monitor, the young mother’s vitals flatlined into a long, agonizing, unbroken tone.
